We evaluate Attention-Deficit/Hyperactivity Disorder in children and youth from input provided by primary observers of the student’s behavior by using an evidence-based instrument for male and female students 5 through 17 years of age. A report can be issued that will assist parents and schools in determining the need to consider a 504 Accommodation Plan or an Individualized Educational Plan (IEP) for the child in the school setting.
